   ***Please note: A lymphedema diagnosis (and proof) is needed to be accepted as a recipient. Lymphatic malformation and vascular malformation cannot be the only diagnosis***

-Lymphedema Diagnosis from doctor

-3 to 5 photos of patient (at least one photo including face)

-Copy of insurance card (if applicable)
